UT DALLAS BLUES AND JAZZ FESTIVAL – Ticket Request Form
This request form is to be used to request tickets for the UT Dallas Blues & Jazz Festival only.
Filling out this form does NOT guarantee you tickets for the event.
You MUST give us a phone number where you can be reached during Box Office the hours of 2-5 p.m. Monday - Friday.
You will receive a call from one of our representatives to take payment information over the phone. Confirmation calls will be made in the order the requests are received.
Your tickets are NOT reserved until you receive confirmation from one of our representatives.
All tickets will be held at Will Call to be picked up the night of the event.
The Box Office will be closed from December 16 - January 7.
The last day to request tickets using this form is Thursday, January 25.
