The Computer Assistance Request Form is to be used
by A&H faculty and staff only.

All issues must be first addressed by utilizing a form
to inform your Arts & Humanities Computer
Technician that you need assistance. To choose
the correct form to fill-out, you need to determine
the importance of your computer assistance request:
A Low Priority call
is one that does not impact the way you work and
has no immediate importance. An example of this
type of call would be fixing your computer’s
sound or changing the wall paper on your Outlook Mail.
A Normal Priority call impacts the way your work, but does not prevent
you from working.
An example of this would be that your printer did
not work (but you have other printers you can access)
or that you have problems receiving e-mail on your
machine (but you can receive it from someone else’s
machine, or email does not play an important role
in your duties).
A High Priority call
directly impacts your ability to do work at all.
An example of a High Priority Call would be that your
video card or monitor went out (you cannot see anything
on the screen) or that your machine had no network
access. In addition to these type of calls, all Virus
Infections are High Priority Calls. Any issue that
directly impacts the ability for multiple employees
to work is a High Priority Call.

needs to be resolved immediately (i.e. an issue that
affects more than one person or an entire division
